Registration Instructions:
**Any NEW player 9 years or older will need to contact the registrar at registrar@flamboroughhockey.com**
1. Logon or Create new account
2. Click on “Register New Participant”
3. Under “PREVIOUS REGISTRATIONS” you will see your child’s name and select “REGISTER NOW”. If your child is not listed, select “REGISTER NEW PARTICIPANT”
**NOTE: Do not put anything in Hockey ID Box complete all other boxes and follow all steps to enter Player/Goalie information
4. Registration Type: Select “PARTICIPANTS”
5. Division Selection: Select the displayed Hockey Division
6. Registration Package: Select Player or Goalie
7. Input Contact Info: All boxes with “*” must be completed
8. Add any Optional Fees that might apply to the registrant. Optional fees include non-city of Hamilton residents and family discount. Click on the next button to proceed to the question section
9. Click Review: Make any corrections if prompted
10. If you wish to register another Player/Goalie, select “ADD TO CART”. Once registration of player(s) is complete, you must click on “CHECK OUT”
11. Select payment option (you have the option to pay either by credit card or cheque)
12. Print Receipt: For income tax purposes (for following year)
13. After printing receipt, logout
14. Payment by credit card: Fill out your credit card details then click the PROCEED WITH PAYMENT button. Wait for your credit card to be processed- DO NOT CLICK ANY BUTTONS UNTIL YOU SEE YOUR RESPONSE. If you transaction is DECLINED, you may enter information for another credit card or choose another form of payment. If APPROVED, you will receive a printable receipt in the next step (You are now done). **** THE FOLLOWING WILL BE DISPLAYED ON YOUR CREDIT CARD STATEMENT: “Flamborough Hockey Association”
15. Payment by cheque (4 installments- July 1st: $100, August 1st: $100, September 1st: $100, October 1st: Remaining amount
A. Print the receipt produced by the system in the next step and deliver it along with payment to your league
B. All four cheques must be into the registrar on or before June 30th, 2012. Failure to do so, registration will be deleted. All NSF cheques are subject to a $25 charge each
C. Cheques MUST be mailed to: Flamborough Hockey Association, P.O Box 1393, Waterdown, Ontario L0R 2H0
***NEW THIS SEASON, IF YOU WOULD LIKE TO BE PART OF THE BENCH STAFF FOR EITHER REP OR HOUSE LEAGUE HOCKEY YOU MUST REGISTER ONLINE. THIS IS DONE THE SAME AS YOU WOULD REGISTER YOUR SON/DAUGHTER. THERE IS NO FEE FOR THIS,WE WANT TO BE ABLE TO TRACK OUR BENCH STAFF VOLUNTEERS. IF YOU HAVE NEVER VOLUNTEERED FOR THE FHA OR ANY OTHER ASSOCIATION YOU WILL NEED TO CONTACT THE REGISTRAR TO CREATE A PROFILE FOR YOU BEFORE YOU CAN REGISTER. IF YOU HAVE BEEN A VOLUNTEER FOR ANOTHER ASSOCIATION IN THE PAST YOU WILL NEED TO FILL OUT A CONVERSION FORM FOUND ON THE OMHA WEBSITE UNDER "FORMS" AND SUBMIT TO THE OMHA.

Refund Policy
Before season starts: full refund less $40 administrative fee
During October: 90% less $40 administrative fee
During November: 75% less $40 administrative fee
** No refunds will be issued after December 1st. Please note that Rep Fees are non-refundable.
Acceptance Policy:
Registrations are accepted when payment has been received in full and registrants are considered members in a good standing with the association.
Privacy Policy:
The Flamborough Hockey Association abides by all Privacy Laws in Canada and does not share its membership information with any third party.

Family Discount: The FHA will discount families with 3 or more children at $100 for the 3rd, 4th and 5th child. The discounts will be applied automatically. In circumstances where fees would make your child’s participation impossible, help may be available. Please make your enquiries to Kim Smith at registrar@flamboroughhockey.com
